Coordinated Bed Access

Coordinated Bed Access (CBA) is the single point of access for post-acute rehabilitative and palliative bedded levels of care, including:

  • Rehabilitation (stroke and general);
  • Low Intensity Rehabilitation;
  • Complex Medical Management;
  • End-of-life/Pain & Symptom Management Complex Continuing Care; and
  • Residential Hospice.  

Standard program criteria, application form and discharge planning approach are used by all sites to facilitate flow of patients.

Hospice Care – Palliative Care Program Complex Continuing Care

The Palliative Care Program provides a range of palliative care to meet the needs of patients with both oncological and non-oncological care needs which include pain and symptom management through to end of life care.
